Donate and register
for an invitation to a reception at the Manhattan home of Amy Tan,
May 18, 2005, 6-8 PM. To register for the May 19 event click
here.
Only
those who have donated and received a confirmation from us will be
admitted. Confirmation will be made on a first-come first-serve basis,
because space is limited. If the guest limit has been reached, your
money will be returned.
The reception will be hosted by Amy Tan to benefit the Lyme Disease
Association. It is by invitation only from Ms. Tan, for donors
at or over $200/individual or $300/couple.
There are two ways to donate and receive a confirmation that you
have been admitted to Ms. Tan’s event:
1) USE A CREDIT CARD
For couples who register by Paypal, we will obtain the second guest’s name from you by email if we confirm your admission. We will also provide the event location at that time and answer any questions.

2) SEND A CHECK
Please make it payable to Lyme Disease Association and mail to:

Because space at Ms. Tan’s home is limited, confirmations will be issued first-come, first-served, based on the date check is received. Paypal is faster. In the event we are full by the time we receive your check, it will be returned to you. Please remember to include your name(s), address, telephone, email, and fax, if any, with your check.
Donations to Lyme Disease Association’s Literati
with Lyme are fully tax-deductible to the extent allowable by law.
The non-deductible portion of each reception ticket is $18. Please
inquire if your deduction is eligible for a matching gift from your
employer and will go to the Lyme Disease Association to support its
public education and research efforts on causes and cures of Lyme
disease, including the Endowed Columbia Lyme Disease Research Center.
All donations to the Lyme Disease Association are fully tax deductible.
A copy of its latest annual report may be obtained, upon request,
from the organization or from the Attorney General, Charities Bureau,
